Larry's Forms
There are 4 forms available here. If you are meeting individually or as a couple with me I ask that you fill out the Life History Questionnaire. If your child is meeting with me I ask that you fill out the Child History Questionnaire. The Consent to Release Information Form only needs to be completed if you wish me to have contact with another treating professional or person. The Telehealth Consent Form is to be filled out if you are wishing to have in part or in entirety treatment through video conferencing or telephone.
These forms can be completed online and printed out, or printed out and filled out. They are not able to be emailed. I would rather you take your time and be thorough than to rush. Thank you.
